Independent Contractor (1099) Acknowledgment & Agreement

I understand and acknowledge that submission of this application does not constitute an offer or contract of employment. If accepted, my relationship with G11 Security Workforce, Inc. (“G11 Security”) will be that of an independent contractor (1099), unless G11 Security provides a different written classification.

Engagement

My engagement with G11 Security is voluntary and may be terminated by either party, subject to applicable law and notice requirements.
I am not guaranteed assignments, compensation, continued membership, or acceptance by submitting this application.
No representative may make an agreement contrary to these terms unless it is written and signed by an authorized company officer.

Pre-membership conditions

I understand that acceptance may be contingent upon completing orientation, any required assessment, a medical or physical examination and drug screening, and submitting a complete application and supporting documents. False, misleading, or omitted information may disqualify me or end a subsequent relationship with G11 Security.

Policies, property, liability, and technology

I will comply with company policies, procedures, post orders, and lawful operational guidelines.
I may be responsible, to the extent permitted by law, for damage caused by my violation of company policy or procedure.
Authorized amounts lawfully owed by me may be deducted from compensation only as permitted by applicable law.
G11 Security may use mobile applications for reporting and guard control. I will maintain working cellular service and keep required duty-related GPS functions enabled while on duty.
I understand that personal cellular service is not compensated unless G11 Security agrees otherwise in writing or applicable law requires reimbursement.

Authorization and release

I authorize G11 Security to verify my education, employment history, professional references, identity, work eligibility, licenses, and other information relevant to this application. I authorize persons and organizations contacted for those purposes to provide responsive information as permitted by law. A copy of this authorization may be treated as valid as the original.

I understand that submitted materials may be retained as part of G11 Security’s business records and may be disclosed when required by applicable law. Disclosure of my Social Security number is used for identity verification, background screening, tax or payroll onboarding, and other lawful reporting purposes.

Photo and video consent

I authorize G11 Security to photograph or record me for identification, training, safety, documentation, and legitimate company business purposes. Unless prohibited by law or otherwise agreed in writing, I understand that company-created business materials are owned by G11 Security and do not create a right to additional compensation.

Pre-membership drug testing acknowledgment

I understand that safety-sensitive assignments may require a drug test and that assignment may be contingent upon a verified satisfactory result, consistent with applicable law and company policy.

Confidentiality and non-disclosure

I may receive confidential or proprietary information, including business practices, security procedures, customer information, pricing, operational data, and financial information. I will not disclose, misuse, copy, or exploit confidential information during or after my relationship with G11 Security. When the relationship ends, I will return company property and confidential materials and retain no unauthorized copies. G11 Security may pursue remedies permitted by law for a breach.

Notice and post responsibilities

I understand that G11 Security requests fourteen days’ written notice before voluntary resignation. I will not abandon an assigned post, leave without authorized relief, sleep while on duty, or fail to report an absence. Compensation adjustments, if any, will be made only when consistent with the work actually performed and applicable law.

Non-discrimination

G11 Security Workforce, Inc. does not unlawfully discriminate in application review, membership consideration, assignments, or service provision based on any status protected by applicable law.

Applicant Privacy Notice

G11 Security collects the information submitted in this form, including contact, identity, Social Security, work-authorization, background, education, employment-history, reference, signature, and uploaded-document information. It is collected to evaluate this application, communicate with me, verify qualifications and identity, conduct authorized screening, document consent, establish an employee or contractor record if approved, meet payroll and tax obligations, protect company and client operations, and comply with legal requirements.

Application information may be accessed by authorized G11 Security administrators and service providers that support hosting, email, screening, legal compliance, payroll, or information security. It may also be disclosed to government authorities, courts, advisers, or other parties when required or permitted by law. G11 Security does not sell applicant information.

Information will be retained for the application process and for a reasonable period afterward based on hiring, recordkeeping, dispute, audit, tax, security, and legal requirements. Approved applicants’ relevant information may become part of their ongoing personnel or contractor record. G11 Security uses administrative and technical safeguards, including encrypted transmission and encrypted storage for the Social Security number, but no internet system can guarantee absolute security.

I may contact G11 Security to ask about my application information or request a correction, subject to identity verification and applicable recordkeeping requirements. By submitting this application, I acknowledge this privacy notice and authorize G11 Security to process the information for the purposes described above.
